It’s now been confirmed a tragic incident on the Mitchell Freeway near Butler, which cost the life of a 60-year-old grandfather late on Wednesday night, was a head-on collision.

One of the first people on the scene James King told Simon Beaumort it’s not hard to imagine how the incident occurred.

“But I think like having the entry and the exit so far apart. I’m not sure if that plays a part in it, and obviously, that late at night, there wouldn’t be many cars around. You’d just be an autopilot,” he said.
